Please export a CSV or EXCEL list of contacts from your CRM with the following information:

First Name
Last Name

Email
Phone Number (if you have multiple numbers for each contact, please include them all)

We recommend sending the SMS Re-activation Campaign offer to your entire list. However, if you have any trouble patients who you would rather not contact - please remove them BEFORE you send us the file
